Notes for completing a registration:
- When registering a child under 18, you are entering your child's name as the member with the parent/guardian name(s) phone numbers and details as primary and secondary contacts.
- If you are over 18, you are the primary contact, selecting member for the relationship to member category.
- Don't have a home phone? Enter as n/a.
- When completing this registration, you are giving consent to Carine Calisthenics Club and Calisthenics Association of WA (CAWA) to use your / your child's name, image, likeness and performances, at any time, to promote the sport of calisthenics by any form of media including print, radio, television or social media platforms. You can revoke this consent by selecting 'No' to the Consent to Publish question. If selecting No, please provide circumstances where consent may be given e.g. training videos on access-limited sites.
